MDLand Speaks at Health Forum on Innovation in Hypertension Management
Innovation in Hypertension Management, the forum hosted by Healthfirst and NYU-CUNY Prevention Research Center at the Grand Hyatt New York on September 20, 2018 was deemed a success and received glowing feedback from its audience of payers, academic researchers, public health/DOHMH, NYU, and community-based providers.
Healthfirst joined public health, community health, and delivery system partners on how multi-sector partnerships can be leveraged to achieve population health, with a specific focus on hypertension. As Dr. Susan J. Beane, MD, Vice President and Medical Director at Healthfirst previously quoted in the September 2017 issue of Spectrum of Health: Improving Outcomes for Hypertension,
"We are on the frontlines of patient care, encountering and serving individuals and groups of patients, families, and communities that may be appropriate for interventions, but the tools for screening, identification, referral or management can be tough to incorporate into daily practice. The challenge is to impact the social determinants in a pragmatic, meaningful, and efficient way, with measurable impact on key health outcomes."
Hypertension, a disease affecting 43 million adults in the United States, 95% of whom have “essential” hypertension with no identifiable and treatable cause, brings us to a heightened focus on improving outcomes. MDLand’s co-founder and Sr. Vice President, Mr. James Long was invited to speak on digital health innovation on population health management. James discussed how providers can leverage their EHR infrastructure by applying various digital technologies to optimize workflow, enable team-based approach, and engage patients to close the care gap and improve the overall population health management, including hypertension and other chronic disease conditions.
MDLand International, one of the major EHR platforms in NYC has engaged in several high-profile research projects and grants by both federal and state public health agencies and academic centers past and present with the focus to improve, prevent, detect, evaluate and manage health outcomes across sectors. The outcome was highlighted at the Agency for Healthcare Research and Quality (AHRQ) meeting.
